GIFT AID DECLARATION

This declaration confirms my wish to make donations to the Dominic Barker Trust under the new Gift Aid Scheme, and applies to donations made by me since 6th April 2002, and to all future donations.

I am a UK taxpayer and want the Dominic Barker Trust to claim back the tax on my gift (reclaiming adds nearly 30% to its value). I confirm that I pay Income and/or Capital Gains Tax at least equal to any tax that the Trust claims on my donations. I will advise the Trust if this situation changes.

Notes

1. You must have paid an amount of income tax and/or capital gains tax at least equal to the tax that The Dominic Barker Trust reclaims on your membership subscription and donations in the tax year (currently 28p for each £1 you give).
2. Please tell The Dominic Barker Trustif you change your name or address.
3. You can cancel the declaration at any time by notifying The Dominic Barker Trust– it will not apply to donations you make on or after the date of cancellation or such later date as you specify.
4. Remember to tell us if you no longer pay an amount of income tax or capital gains tax equal to the tax we reclaim on your donation.
5. If you pay tax at the higher rate you can claim further tax relief in your Self-Assessment tax return.
